One of the main challenges for health professionals is the development of skills in the use of Information and Communication Technologies; a support tool for prevention, diagnosis and treatment, mainly in highly prevalent chronic pathologies.
The frequency of Chronic Kidney Disease is increasing worldwide. In Uruguay it affects 7% of the population. It is recommended to assist patients in stages 1 to 3a in Primary Health Care through a referral/counter-referral system with a nephrologist. Follow-up of stages 3b to 5 by the nephrologist. Adequate nephrological care is limited due to lack of specialists or their concentration in the capital city.
Telemedicine incorporates digital connectivity to the specialty, facilitating remote follow-up and communication between health teams.
Objective: to improve accessibility to nephrological consultation of patients from the interior of the country assisted by Primary Health Care physicians, using electronic medical records and Information and Communication Technologies.
Methodology: joint follow-up of patients from the interior of the country, by Nephrology and Primary Care physicians in a referral-counter-referral system, remotely in synchronous and asynchronous modalities, individualized by patient, control to be followed (face-to-face or remote) and frequency. The feasibility of the Project was evaluated with the Communications and Information Technology Division of the National Health Directorate and the General Directorate of Peripheral Care.
Conclusion: the Project is feasible through the integration of available human and technological resources, ensuring confidentiality and protection of patient data according to the current legal regulatory framework.
