Severe mandibular prognathism.

A clinical case of mandibular prognathism is presented, particularly severe because of its dimensions and because the patient is edentulous, which aggravates his prognathism condition. The clinical and radiographic study is reported, the models are analyzed and the cephalometric diagnosis is made. The applicable surgical techniques are discussed and it is decided to perform the Obweggesser sagittal osteotomy technique. An acrylic splint is designed and used as an intraoperative guide and as an artifice for postoperative fixation. The patient is treated in two stages. During the intraoperative surgical correction of the prognathism, an anatomical particularity appears that requires an operative variation. Two different techniques are combined in each of the branches of the lower jaw. The postoperative result achieved with a satisfactory intermandibular relation from the functional and esthetic point of view, radiographically verifiable, is presented.
