Le Fort I osteotomy as surgical access

Multiple surgical approaches have been described for treatment of maxillary lesions. Horizontal osteotomy of the maxilla at the level of the fracture line, as described by Le Fort in 1901, was originally reported as a surgical approach for the resection of nasopharyngeal tumors in 1859. Since the 1970s, this procedure has been popularized in orthognathic surgery for the correction of dentofacial deformities. The Le Fort I type osteotomy was ignored as a surgical approach for several decades, regaining popularity in the late 1980s especially for access to skull base, facial center and nasopharyngeal tumors. The main advantage of this procedure is that it offers a panoramic view of the maxillary sinus and nasal cavity, allows complete exposure and facilitates the complete removal of large lesions without the need for unnecessary extensions and resections of the maxillary bone. We present the case of a maxillary odontogenic keratocyst using the Le Fort I osteotomy as the surgical approach for its treatment.
