Pericardial teratoma is a rare clinical entity that predominates in children, whose origin is aberrant nests of multipotent cells for the three germ layers. Its clinical presentation may be that of a radiological finding, pericardial complications, including cardiac effusion and tamponade, as well as respiratory alterations of variable intensity that may reach respiratory failure. Given the clinical interest of this condition, we present a case of a 3-year-old girl who consulted for a respiratory condition due to cough, dyspnea, fever and moderate retrosternal pain. Computed Axial Tomography (CT) of the thorax was compatible with a mediastinal teratoma. Complete excision by median sternotomy of the bulky intrapericardial tumor was performed, revealing a benign teratoma of the pericardium. Six years later the patient is still free of disease.
