Cavernous sinus thrombosis

Material and methods: We present the clinical case of a 63-year-old patient who consulted for right fronto-orbital headache and fever. He added vomiting and diplopia in the right eye, confirming on examination paresis of the right external rectum without chemosis or pain. There were no alterations in the initial paraclinical examination (imaging and laboratory techniques), and 3 days later she developed right proptosis, bipalpebral edema and flushing with increased headache and nausea. Magnetic Resonance Imaging (MRI) of the orbit reports orbital and right periorbital cellulitis, thrombosis of the superior ophthalmic vein and cavernous sinus. Blood cultures were positive for Streptococcus viridans.
Results: treatment with broad-spectrum antibiotics was performed, with coverage for S. aureus and anticoagulation, not requiring surgical treatment, with good evolution without ophthalmologic sequelae.
Conclusions: cavernous sinus thrombosis is the final complication of orbital cellulitis, which requires early detection for timely treatment, sometimes requiring surgery.
