Probiotics in caries prevention

Research on the application of probiotics in the oral sphere is quite recent, and concerns the prevention and treatment of caries, periodontal disease, halitosis and candidiasis. Initially they were introduced in the dairy industry, and today the most used strains belong to lactobacilli and bifidobacteria. The mechanisms of action are not yet clearly known, although they seem to derive from a mixture of resistance to pathogen colonization with modulation of the immune response. There is evidence that probiotics may help control caries disease due to their ability to inhibit cariogenic streptococci. The fact that some probiotic bacteria are involved in the initiation or progression of caries lesions has sparked discussion about the safety of their use as part of preventive or therapeutic caries control programs. Further research will be needed in this area to select the most appropriate strains, to elucidate their mechanisms of action and to determine the optimal doses and the most appropriate vehicles. Although no clear recommendations on its use can be given to the practitioner at present, the evidence indicates that this therapy may become a reality in the future of dentistry.
